Technical Assistance:
We discussed the requirements for staff records, behavior management, formula records, nap linens, hand washing and hot water temperature. The risk assessments for violations was provided. We talked about methods for documenting emergency drills.

Comments:
Thank you for your assistance during the unannounced monitoring inspection conducted from 10:39 AM to 4:00 PM. Today, there were fifty children, ages infancy through five years, with five to seven to eight staff. I viewed program activities, daily routines, staffing, supervision, interactions with children, equipment, infant care and records, behavior management, injury records, emergency supplies and drill records, posted information, menu/nutrition, diapering procedures, five records for children, five staff records, staff qualifications, staff training, indoor and outdoor areas. The pre-K children were preparing for graduation this week. The teachers have planned regular group activities such as stories, music and art/crafts. They enjoyed active play outside. Please, review the minimum standards and the subsidy vendor requirements. We discussed the importance of maintaining compliance with minimum standards for health and safety. Violations:
Standard #: 22VAC40-185-70-A
Description: Based on a review of five records for staff, and interviews with administrative staff, the center failed to write the effective dates for changes with position titles for one staff member. Evidence: Staff member 2 was observed working as an lead teacher. Administrative staff indicated that staff member 2 began working with the children as a foater/subsitute in March 2019 and then as a lead teacher on May 11, 2019. However, the changes for positions worked at center since July 2, 2019 were not documented in the staff record.

Plan of Correction: An administrator will document the correct dates and position titles in the staff record. Whenever a staff position title changes, it will be written in the staff file at the time of the change.

Standard #: 22VAC40-185-320-B
Description: Based on the measurement of the hot water temperature at a sink used in a bathroom area and diaper changing station, and interview with staff, the center failed to ensure that the hot water used by children did not exceed 120 F degrees. Evidence: The licensing inspector turned on the hot water at the sink in the back hall area used by the youngest children after toilet use, during meals/snacks and for diapering of toddler and two year old children. The inspector immediately felt the rise in hot water temperature. According to a thermometer used to measure the temperature, it reached 124 Fdegrees

Plan of Correction: The administrator turned down the temperature control on the hot water heater the evening of the inspection. The hot water temperature will be checked any time an adjustment is made.

Standard #: 22VAC40-185-410-1
Description: Based on observation of the infant area, and interviews with staff, the center failed to ensure that staff did not use physical methods of behavior management and discipline. Evidence: At 2:40 PM, while the inspector was standing in the central corridor and observing the nap time in the toddler area while also watching the infant area through the glass window, a thirteen month old, child 6, was seen on top of a Fisher Price infant seat with her hands on the back of the seat while she looked through the window toward the toddler area and was adjusting her feet and legs on the seat. Infant 6 leaned forward and the chair back rocked downward. The inspector saw staff member 2 next to the infant and facing the changing table, so her side was to the infant. The inspector waved a hand to staff member 2 and staff member 6 in the toddler area knocked on the window. Both the inspector and staff member 6 pointed to infant 6. Staff member 2 was seen turning to the infant and using her right open hand to make contact with the right side of the infant's hip and upper leg area. The infant began to cry. Next, staff member 2 lifted the infant off of the chair and moved her to the floor. Then, staff member 2 moved the chair upside down onto the floor. Next, child 6 sat on the bottom shelf of the changing table and continued to cry. The inspector entered the infant room and did not see any toys accessible for the infant's floor time.

Plan of Correction: The administrator immediately addressed the action taken by staff member 2 and options for infant activities so they did not try to climb onto infant seats. The administrator will arrange for staff member 2 to participate in training about age appropriate infant care and behavior management. An administrative staff person will be assigned to work along with staff member 2. Staff member 2 will be a floater and substitute when a new infant teacher is assigned to the room. The staff will be instructed to never use any physical contact with children while addressing a behavior concern.

Standard #: 22VAC40-185-450-A
Description: Based on observation of children during rest time, and interviews with staff, the center failed to ensure that all children using cots have all required linens. Evidence: Seven of thirteen children laying on cots in the older preschool area were directly on cots and did not have a bottom cover. A second blanket was provided by the center staff that was either used as another top cover, folded for a head pillow, or laying to the side. Larger blankets with some of the children were used only as top covers and were not in use as both a bottom and top cover.

Plan of Correction: The teachers will be told to make sure children do not move the bottom cover placed on the cots and use them for any other reason such as a top cover or folded as a pillow. The teachers will let the children know they are to leave the bottom covers on the cots. The administrative staff will check on the use of bottom covers for children using cots for nap time.

Standard #: 22VAC40-185-500-A
Description: Based on observation of the routine after lunch, and interviews with staff, the center failed to ensure that all children washed hands after eating a meal. Evidence: As ten children, ages two to three years, finished eating lunch at a table in the entrance area of the center, five children were seen leaving the table and entering the two year old area where they were directed to cots. At least two children were told to fetch personal items at a cubby and to go to a nap area. One child finished eating, went to the bathroom, and then went to the middle corridor to wait at the room divider for the toddler area. After waiting a few minutes, a teacher told her to go to the toddler area and a cot for lunch.

Plan of Correction: The teachers will have a routine of directing children from the table to the sink for hand washing. If children use the toilet after lunch, a teacher will check with the child about hand washing before they go to a cot for nap time. The teachers will give reminders to the young children about hand washing after they finish eating. Administrative staff will review the licensing standard with staff that requires hand washing for children.

Standard #: 22VAC40-185-570-C
Description: Based on a review of records for children on formula at the center, and interviews with staff, the center failed to obtain all required written information for infants from parents Evidence: When asked about the record for two infants fed formula at the center, the staff could not locate a record indicating the brand of formula, amount and schedule for feeding. Staff indicate they have been told verbally about feeding infant 1F and 2F formula in bottles. An infant feeding schedule with the required record was not located in the children's files.

Plan of Correction: The administrative staff will have the parents provide written information about the brand of formula, amounts to feed and the schedule. The administrative staff will make sure a feeding schedule form is provided to parents at the time of enrollment.
