An unannounced complaint investigation was completed on 6/21/2019 in response to a complaint received by the licensing office on 5/9/19. Record review, classroom observation and interviews with staff were conducted relating the allegation regarding injury reporting 6/21/19. The information gathered during the investigation supports the allegation of injury reporting, so the complaint is determined to be "valid". Standard #: 22VAC40-185-550-M Complaint related: No Description: Based on record review and interview it was determined that the center did not maintain complete written records for all children's injuries. Evidence: 1. The injury report for child #6 dated 4/3/19 did not contain the time of the injury, and the date, time and method of parental notification. 2. The injury report for child #7 dated 4/3/19 did not contain future action to prevent recurrence, and the date, time and method of parental notification. 3. The injury report for child #8 dated 4/3/19 did not contain future action to prevent recurrence, first aid treatment given, and the date, time and method of parental notification. 4. The complainant reported "my child came home with a cut on their face which was never documented and I was never advised as to how it occurred". Plan of Correction: Director will retrain staff on reporting all required information on the injury reports.
