Comments:
A supplemental health and safety inspection was completed on May 24, 2019 from approximately 9:45a.m. to 11:00a.m. to review supplemental health and safety requirements. The inspection was initiated in response to specific compliance requirements the department was made aware of while investigating a complaint. Staff, children, a classroom, and the playground were observed and staff and collaterals were interviewed. The Director and Owner were available for the inspection and were present for the exit interview at which time the inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Standard #: 22VAC40-665-550-2
Description: Based on interviews with staff, the licensee failed to inform the department's representative as soon as practicable, but not more than two business days, of any injury to a child while under the vendor's supervision when a referral is made for treatment from a medical professional. Evidence: 1. Staff #1 reported the center was informed by Child #1's parent that Child #1 obtained an injury at the center on April 22, 2019, in which medical treatment was obtained on April 23, 2019. Staff #1 reports the parent notified the center about the medical treatment on April 23, 2019 and that the center did not report the injury to the Division of Licensing per the requirement.

Plan of Correction: The Director will report all injuries that require medical treatment to the Division of Licensing per the requirement.
