Standard #: 22VAC40-185-550-M Complaint related: No Description: Based on review of injury records, not all records are complete. Evidence: Injury report for child #1 was incomplete. The record contained the wrong date and time of incident, time parent was notified, and a parent signature or two staff signatures were not documented on the form. Plan of Correction: I will review the process of completing the incident/accident reports and make sure they are competed correctly.
