63.2(17) License & Registration Procedures

Technical Assistance:
The license cannot be issued until the Initial Application is deemed completed. The following items are required in order to complete the application: Certificate of Occupancy/Building Evaluation, Fire Inspection, Health Inspection, all remaining background search findings. Staff shall maintain their class attendance on their person at all times when outside of their base classroom.

Comments:
An announced inspection for initial licensure was conducted today. There were 90 children in care, with a total of 17 staff members assigned to their direct supervision. Children's records, staff records, background searches, staff training, the indoor/outdoor physical plant, emergency preparedness items, and administrative documents were inspected today. The children were observed playing group games, engaging in classroom Montessori activities, eating lunch from home, and interacting with staff members. This inspection was completed between approximately 10:00am and 12:30pm. Thank you for meeting with me today. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, it was determined that children's records are not complete. Evidence: 1 - Children's records do not request each child's home address. 2 - Child record A and D do not include information for each child's "authorized pick ups."

Plan of Correction: All records will be updated. The home address section will be updated on the registration forms.

Standard #: 22VAC40-185-270-A
Description: Based on observations, it was determined that various areas of the school have not been maintained as required. Evidence: 1 - The children's nap mats are torn in various places. 2 - In Room 2, the children's toilet seat has broken. This toilet cannot be used by the children. 3 - There are two holes in the resilient surfacing beneath the large set of playground swings.

Plan of Correction: 1 - New rest mats will be ordered and all old rest mats will be disposed of. 2 - Maintenance has already been contacted to replace the toilet seat. Maintenance will be contacted again. 3 - The holes will be repaired.

Standard #: 22VAC40-185-520-D
Description: Based on a review of children's topical ointment products, it was determined that staff do not record the application of insect repellent as required. Evidence: Staff do not currently record when insect repellent is applied to children. In addition, one child's container of insect repellent was not labeled with the child's name.

Plan of Correction: Staff will begin documenting when bug spray is applied to children. The bug spray bottle will be labeled today.

Standard #: 22VAC40-185-560-N
Description: Based on observations, it was determined that children were permitted to walk around while drinking. Evidence: The Inspector observed various Toddler children walking around drinking from water bottles while playing in the Great Room.

Plan of Correction: All staff will be reminded to make sure children are seated to eat and/or drink.

Standard #: 22VAC40-191-40-C-1-A
Description: Based on a review of the initial application, it was determined that two required VA background searches have not yet been fully completed. Evidence: Board member A and Board member B are listed on the initial application. Both of these individuals are required to complete a Virginia Child Protective Services (CPS) background search. The findings for each of these searches have not yet been submitted to the Licensing Inspector.

Plan of Correction: For each individual the search request has been submitted, but the results have not been received. Once the results are received, the findings will immediately be forwarded to the Inspector.

Standard #: 63.2(17)-1720.1-A
Description: Based on a review of staff background checks, it was determined that one staff person was permitted to work in her classroom role prior to the receipt of one required background search. Evidence: Staff person A has a documented date of hire of 6/17/19; however her fingerprint search results were not received until 6/20/19.

Plan of Correction: Moving forward no staff will be permitted to work in their classroom role until the fingerprint search results are received.

Standard #: 63.2(17)-1721.1-B-3
Description: Based on a review of the initial application, it was determined two required out of state background searches have not yet been fully completed. Evidence: Board member A and board member B are listed on the application. Based on residency, both of these individuals are required to complete an out of state CPS background search. The findings for each of these searches have not been submitted to the Licensing Inspector.

Plan of Correction: Both search requests have been submitted, but the results have not been received. When the results are received, the findings will be immediately forwarded to the Inspector.
