Comments:
An unannounced inspection was conducted today, June 18, 2019 in response to a complaint received by The Department of Social Services, Licensing on June 14, 2019 concerning supervision and preventing the spread of disease. The inspection was initiated at 2:30 pm and concluded at 4:45 pm. Staff interviews were conducted and documentation of policies and procedures reviewed. Based on the information gathered during the investigation, the preponderance of evidence supports the portion of the complaint regarding supervision, therefore the complaint is valid. Standard #: 22VAC40-185-340-C
Complaint related: Yes
Description: Based on interview, the center did not ensure during the stated hours of operation, there was on the premises and on field trips when one or more children are present one staff member who meets the qualifications of a program leader or program director and an immediately available staff member, volunteer or other employee who is at least 16 years of age, with direct means for communication between the two of them. Evidence: 1. Staff #1 stated on June 11, 2019 she walked alone with child #1 from the Central Montessori Infant and Toddler building to another licensed location, the Central Montessori School building, alone approximately 0.1 miles away. 2. The two buildings are not adjacent to each other and require crossing two streets that intersect.

Plan of Correction: Per the Owner: Before this DSS visit, Central considered walking children between buildings to different classrooms to be a transitional event, not a field trip. Infant and toddler staff have now been trained on field trip protocols, ensuring that two staff members walk a child/children between classrooms. No other field trips are permitted for this age group.

Description: Based on interview, the center did not ensure a first aid kit was accessible on field trip and wherever children are in care. Evidence: 1. Staff #1 stated while walking child #1 from the Central Montessori Infant and Toddler building to another licensed location, the Central Montessori School building, on June 11, 2019, approximately 0.1 miles away, she did not carry a first aid kit. 2. The two buildings are not adjacent to each other and require crossing two streets that intersect.

Plan of Correction: Per the owner: First aid kits are always located in all classrooms. Protocols for field trips will be followed ensuring that a first aid kit accompanies a child or children during transitions.

Description: Based on interview, the center did not ensure there was a communication plan between center staff and staff who are transporting children or on a field trip. Evidence: 1. Staff #1 stated on June 11, 2019, she did not have a means of communicating with the center while walking alone with child #1 from the Central Montessori Infant and Toddler building to another licensed location, the Central Montessori School. 2. The two buildings are not adjacent to each other and require crossing two streets that intersect.

Plan of Correction: Per the owner: All transitions between classrooms are planned and will continued to be planned with parental and administrative notifications. After this DSS visit, e-mails will be sent between campuses before child begins transitions and when they return. A teacher will carry a walkie-talkie and or cell during transition.
