Technical Assistance:

The license cannot be issued until the initial application is deemed complete. The following items are still required in order for the application to be deemed complete: Certificate of Occupancy/Building Evaluation; Fire Inspection, Health Inspection, all remaining background search findings. Staff CPS clearances that were requested under the new school name have not yet been received. When they are received, the Director shall forward the requested findings reports to the Licensing Inspector.

Comments:

An announced full inspection for initial licensure was conducted today. There were 26 children in care, with a total of 6 staff members assigned to their direct supervision. Children's records, emergency preparedness items, administrative documents, classroom materials, the indoor/outdoor physical plant, children's medications, and classroom programming were inspected today. The children were observed playing water games outside and enjoying outdoor playground time. This inspection was completed between approximately 10:00am and 11:30am. Violations:

Standard #:	22VAC40-185-60-A
Description:	Based on a review of children's records, it was determined that three records are not complete. Evidence: 1 - Child record A and record C do not include each parent's place of employment information. 2 - Child record B does not include "authorized pick ups" information.
Plan of Correction:	The registration forms have been updated since these parents completed them. Each parent will be asked for the missing information.
Standard #:	22VAC40-185-80-A
Description:	Based on a review of administrative documents, it was determined that one classroom attendance was not completed correctly. Evidence: The attendance record for Room 2 indicated a total of 14 children present and in care; however, only 13 children were actually present and in care.
Plan of Correction:	The attendance was corrected. Moving forward, we will begin using an electronic attendance system that requires parents to accurately sign their child in/out each day.
Standard #:	22VAC40-191-40-C-1-A
Description:	Based on a review of the initial application, it was determined that two required VA background searches have not been fully completed. Evidence: Board member A and board member B are listed on the initial application. Both of these individuals are required to complete a Virginia Child Protective Services (CPS) background search. The findings for each of these searches have not been submitted to the Licensing Inspector.
Plan of Correction:	For each individual the search request has been submitted but the results have not been received. Once the results are received, the findings will be immediately forwarded to the Inspector.
Standard #:	63.2(17)-1721.1-B-3
Description:	Based on a review of the initial application, it was determined that two required out of state background searches have not yet been fully completed. Evidence: Board member A and board member B are listed on the application. Based on residency, both of these individuals are required to complete an out of state Child Protective Services (CPS) background search. The findings for each of these searches have not been submitted to the Licensing Inspector.
Plan of Correction:	Both search requests have been submitted, but the results have not been received. When the results are received, the findings will be immediately forwarded to the Inspector.
