Comments:
An unannounced non-mandated monitoring inspection was completed on 06/24/2019 from 10:40 A.M.-1:15 P.M. The inspection was conducted in response to a self-reported incident related to supervision of children on 06/20/2019. One child's records, two staff records, attendance sheets, sign in and out forms were reviewed, parent handbook, employee handbook, and eight staff were interviewed. Violations:
Standard #: 22VAC40-185-40-E
Description: Based on review of policies and procedures and interviews, the operational responsibilities of the licensee did not ensure that the center's activities and services are maintained in compliance with the center's own policies and procedures that are required by these standards. Evidence: 1. The center did not follow their policy for lost/missing child located in the Parent Handbook. The handbook states," If the child is lost or missing from the Center, both the Director and the Building Superintendent will be notified immediately. All staff will be notified and requested to check their rooms for the child. All available personnel in the Center as well as church staff will search the facility in its entirety along with the church grounds and immediate vicinity." 2. On 06/20/2019 there were 23 children, two years-five years of age, on the playground with five staff. 3. Staff #2 was aware Child #1 (3 years) was missing from the playground on 6/20/2019. Staff # 2 stated that she did not notify any staff that a child was missing from the group. 4.. Staff #1 found Child #1 in the restroom playing in water unsupervised. 5. Staff #1 returned Child #1 to the playground, and told Staff #2 where she found Child #1. 6. Staff #3,#5, and #6 verified that Staff #2 did not tell them she had a child missing from her group.

Plan of Correction: Director will hold a staff meeting where staff be retrained on the policy about lost or missing children. It will be held within two weeks of the incident.

Standard #: 22VAC40-185-240-B
Description: Based on a review of the center's written policies and procedures and interviews with administrative staff, staff were not provided in writing with all required information by the end of the first day of supervising children. Evidence: 1. Documentation provided to staff did not contain the following required information; procedures for supervising a child arriving after scheduled classes or activities including field trips; procedures to confirm absence of a child when arriving from another program or agency; procedures for identifying where attending children are at all times, including field trips; procedures for ill or injured children, medical emergencies and general emergencies; policies for administration of medication; and procedures for response to natural and man-made disasters. 2.Administrative staff verified the required policies were not included in the written information that employees are provided upon hire.

Plan of Correction: The handbook will be revised, submitted to the board for approval, and released to the staff and licensing.

Standard #: 22VAC40-185-340-A
Description: Based on interviews, staff failed to ensure the care and protection of a three year old child. Evidence: 1. On 06/20/2019 there were 23 children, two years-five years of age, on the playground with five staff. 2. A three year old child was out of sight and sound supervision of staff for approximately ten minutes. Staff #1 found Child #1 in a restroom in the back hallway washing his hands. No staff were in the hallway or restroom with Child #1. 3. Staff #1 stated she left Classroom A at approximately 4:40 P.M. to gather trash out of the back restrooms. Staff #1 found Child #1 at approximately 4:41 P.M. 4. . Staff #1 returned Child #1 to the playground and spoke with Staff #2. Staff #2 was unaware that Child #1 was in the restroom unsupervised. 5. Staff #2 and Staff #6 stated they recalled seeing Child #1 at 4:30 P.M. when they left the playground to take two children to the restroom. Staff #2 and Staff # 6 verified that Child #1 was not one of the children going in to use the restroom. Staff #2 and Staff #6 do not recall seeing Child #1 when they returned to the playground. 6. Staff #3, #5, and #6 verified that they did not know that a child was missing from the playground until Staff #1 returned Child #1 to the playground.

Plan of Correction: Director will retrain staff on the importance of sight and sound supervision within two weeks of the incident.

Standard #: 22VAC40-185-340-F
Description: Based on interviews, the center failed to ensure that children under 10 years of age were always within sight and sound supervision of staff. Evidence: 1. On 06/20/2019 there were 23 children, two years-five years of age, on the playground with five staff. 2. A three year old child was out of sight and sound supervision of staff for approximately ten minutes. Staff #1 found Child #1 (3 years) in a restroom in the back hallway washing his hands. No staff were in the hallway or restroom with Child #1. 2. Staff #1 stated she left Classroom A at approximately 4:40 P.M. to gather trash out of the back restrooms. Staff #1 found Child #1 at approximately 4:41 P.M. 3. Staff #1 returned Child #1 to the playground and spoke with Staff #2. Staff #2 was unaware that Child #1 was in the restroom unsupervised. 4.. Plan of Correction: Director will retrain staff on sight and sound supervision and the importance of performing head counts, name to face counts, and how often they should occur.

Standard #: 22VAC40-185-420-A
Description: Based on review of the Parent Handbook and interviews, the center failed to provide parents with all required information in writing before the child's first day of attendance. Evidence: 1. The parent handbook was missing notification regarding the custodial parent's right to be admitted to the center and the policy for communication with parents in an emergency situation. 2. Staff #8 verified that the parents were not provided with all required information in writing before the child's first day of attendance.

Plan of Correction: The handbook will be updated, submitted to the board for approval, and released to the parents and licensing.
