Comments:
An unannounced Complaint Inspection was initiated in the center on June 25, 2019, completed on June 28, 2019 regarding a complaint received by the department. Complainant reported concerns related to Special Care Provisions/ Emergencies and Programs. Based on observation, review of photographs and staff interviews, there was not a preponderance of evidence to support the allegation of Staffing/ Supervision (child to staff ratios) and Special Care Provisions/Emergencies (injury reporting) , therefore the complaint has been determined to be NOT VALID. There were violations found that were not reported and those areas of noncompliance are identified on the violation notice. The results of the inspection and violations cited were reviewed and verified with the designated staff member in charge (Toddler Teacher). Description: Based on observation, the licensee did not ensure there shall be at least 12 inches of space between occupied cots, beds, and rest mats. Evidence: The preschoolage and toddler age children were observed during their designated rest period and 7 of the 12 cots/mats observed did not have at least the required 12 inches between them. The space measured between the cots/mats was between 3 and 4 inches.

Plan of Correction: The toddler teacher will make sure there is correct amount of space between each cot and mat and talk with the director to make sure the staff is spacing the cots with at least 12 inches apart.

Complaint related: No
Description: Based on observation and interviews, the licensee did not ensure that children's hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks. Evidence: Seven out of seven children in the toddler room and six out six children in the multipurpose room were observed after eating lunch and they did not wash their hands after eating.

Plan of Correction: Hand washing will be done in every class before and after meals as well as return from outside.
