Technical Assistance:
The issuance of a six month Conditional license is pending until receipt of all background search results are completed and received and receipt of the local fire inspection report.

Comments:
An announced Initial inspection was conducted this date. A total of 45 children present with 15 staff. Observation of all 5 occupied classrooms was conducted, inspection of the outdoor play area and record review which consisted of 6 children's records and 8 staff records was completed. In one class, staff observed reading to the children in a circle. The topic of the book was the Statue of Liberty. Children raised hands to recall details of the book. Some of the children were observed putting on rubber boots to transition to the outdoor playground. Once they transitioned to the outside, staff were observed strategically placed on the playground to provide adequate staff supervision. Staff interviewed regarding emergency procedures and policies. Staff observed assisting some children with the process. Hand washing procedures was observed prior to lunch. Staff were observed sitting with the children during lunch and engaging in conversation. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on the child record review, a physical examination was not completed. Evidence: Child #1 began enrollment on 5/16/19, a physical examination within 30 days of enrollment was not found.

Plan of Correction: Contact with the child's parent will be made and a completed physical exam will be obtained.

Standard #: 22VAC40-185-330-B
Description: Based on observation of the playground area, resilient surfacing was not maintained. Evidence: The area under the climbing area and the area beneath the two slides did not have sufficient resilient surfacing. Less than six inches was maintained.

Plan of Correction: The mulch in these areas will be raked to ensure that a minimum of six inches is maintained. A work order request will be submitted to install a long term appropriate resilient surface for the playground.

Standard #: 22VAC40-185-510-D
Description: Based on review of the medications on site, a medication authorization for an emergency medication is no longer effective. Evidence: Child #6's medication authorization for an emergency medication expired in March 2019.

Plan of Correction: A new authorization for this medication will be obtained by the parent.

Standard #: 22VAC40-191-40-C-1-A
Description: Based on review of the initial application, two required Virginia Central Registry background checks have not been fully completed. Evidence: Board Member #1 and #2 are listed on the initial application. A Virginia Central Registry search results have not been received to date.

Plan of Correction: For each individual a Virginia Central Registry search request has been submitted. Results remain pending. Once results are received, they will be forwarded to the Division of Licensing for review.
