Comments:
An unannounced joint investigation with a local agency was conducted on 04/22/2019 in response to a self reported incident regarding supervision. During the investigation, staff interviews were conducted, records and documents were reviewed, and the physical plant of the indoor and outdoor play areas were inspected. Violations:
Standard #: 22VAC40-185-40-E
Description: Based on records reviewed and staff interviews, staff did not follow the center's own policies and procedures. Evidence: 1. The center's "transition tracking Policy" states that "Teachers are required to use the We Care Transition Tracking Sheet to account for children in their care and conduct a Face to Name Roll Call whenever a transition through a door or gate occurs, followed by a Head Count". 2. On 4/18/2019 at approximately 5:10pm, Staff 1 and Staff 2 walked the Twos-1 and Twos-2 groups from the playground to their classroom. To get to the Twos-1 classroom from the playground, a distance of approximately 50 feet, the group must exit the playground gates, walk down a ram, enter the building, go down a short hallway, and make a left down the main corridor that leads to the Twos-1 classroom on the left. 3. Staff 1 and Staff 2 stated that when leaving the playground, Staff 1 and Staff 2 conducted their Name to Face when entering the building; however, a Head Count was not documented on the tracking sheet at that time. 5. Child 1 ( 2years old) was left alone and unsupervised outdoors for approximately 10 minutes when their group returned to the classroom and until the child was found by a parent who returned to the playground in search of a missing item.

Plan of Correction: A Corrective Action plan was received in our office in the allowable time frame. Please contact the licensing inspector for the responses.

Standard #: 22VAC40-185-340-F
Description: Based on staff interviews, staff documentation, and records reviewed, a child under the age of 10 years was not within actual sight and sound supervision of staff. Evidence: 1. On 4/18/2019 at approximately 5:10pm, Child 1 (2 years old) was left outdoors, out of sight and sound supervision for approximately 10 minutes, when the group left the playground with Staff 1 and Staff 2 to return to their classroom. 2. Staff 1 stated that when hey returned to the classroom, a Name to Face was conducted but that Child 1 was mistaken for their identical twin. Staff 1 further stated that after entering the classroom, a head count was conducted in which they realized Child 1 was missing. Staff 1informed Staff 4 who went to obtain the assistance of Staff 5 to look for the child. 3. Staff 3 stated that while indoors, at approximately 5:20pm, they overheard a parent [of another child enrolled] informing another staff that a child was left outside. Staff 3 immediately rushed outdoors and found the child crying and standing on the digger of a bulldozer parked within a landscaping construction zone in an unsecured outdoor common approximately 45 feet from Gallows Road and approximately 45 feet from the entrance to the building.

Plan of Correction: A Corrective Action plan was received in our office in the allowable time frame. Please contact the licensing inspector for the responses.
