Comments:
An unannounced monitoring inspection was conducted today from 10:16am-1:00pm. There were 95 children (5-12yrs.) supervised by 19 direct care staff. The physical plant, 4 staff records, 10 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed playing games in the gym, eating lunch, and preparing to go on a bowling fieldtrip. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, 2 of 10 children's records did not contain documentation of all required information. Evidence: 1. Child #1's (start date: not documented) record did not contain documentation of dad's place of employment, mom's work telephone number, an address for the second emergency contact person, and the first date of attendance. 2. Child #2's (start date: 5/30/2018) record did not contain documentation of child updates and confirmation of up-to-date information in the child's record as required by 22 VAC 40-185-420 E 3. The most recent documented parental review of emergency contact information was dated 5/30/2018.

Plan of Correction: I will get parents to fill in all missing information.

Standard #: 22VAC40-185-510-A
Description: Based on a review of medication, 2 of 11 medication authorization forms did not contain documentation of parent authorization. Evidence: Child #2's (start date: 5/30/2018) Children's Allergy and Epinephrine authorization forms did not contain documentation of a parent signature.

Plan of Correction: I will have the parents sign the forms today.

Standard #: 22VAC40-185-510-N
Description: Based on a review of medications, 1 of 11 children's medications were not picked up, or returned to the parent, within 14 days of the authorization expiration. Evidence: Child #2's (start date: 5/30/2018) Aleve medication authorization form expired 9/6/2018.

Plan of Correction: We will have the parents update the form.
