Comments:
A self reported incident was called into the Fairfax Licensing Office by the program administration. Based on interviews, documentation and observation, the incident resulted in violations. Violations:
Standard #: 22VAC40-185-40-E
Description: Based on staff interviews and documentation review, the licensee failed to maintain the centers own policies and procedures. Evidence The Cooperative playgroup Handbook (page 17) states that working parents responsibilities include "Be aware of participants? whereabouts at all times and ensure that they are engaged in appropriate activities at all times. All children shall be within actual sight and sound supervision of working parents" Child #1 and Child #2 were able to exit the building and were not supervised by working parents for approximately 3-4 minutes.

Plan of Correction: Parent workers were retrained and provided guidance on the care and supervision of children.

Standard #: 22VAC40-185-340-A
Description: Based on working parent and staff interviews, the working parent and staff were not able to ensure care, guidance and protection of two children enrolled in the cooperative playgroup program. Evidence: 1. Child #1 (age 22 months) and child #2 (age 27 months) were able to exit the classroom and walk out of the community center. Child #1 was found at the top of the outside steps by the working parent. Child #2 continued down the steps and walked to the end of the block, approximately 415 feet away. Child #2 was found by Staff #1 "hugging a tree, very upset and crying mommy" 2. Neither the working parents and the staff did not intervene immediately as the two children were gone from approximately 10:10-10:14 am for approximately 3-4 minutes.

Plan of Correction: Staff retrained parents are ensuring they are counting and keeping track of children. Parents are being supervised by staff to ensure they are not distracted and are monitoring their assigned children and area of supervision. Door will remain closed and a door chime was installed. Additional staff desk was moved next to door to provide additional level of supervision.

Standard #: 22VAC40-185-340-F
Description: Based on staff and working parent members, two children were not always kept in sight and sound supervision. Evidence: 1. At approximately 10:10 Staff #1 and the Working Parent realized that child #1-2 were not in the classroom. They left the room and searched the surrounding space. 2. The Working Parent found child #1 standing at the top of the outside steps. 3. Staff #1 saw child #2 at the end of the block. The distance is approximately 415 feet from the classroom door to the tree where child #2 was found. Both adults stated in the interview that the 2 children were out of sight and sound supervision for approximately 3-4 minutes.

Plan of Correction: Staff retrained parents are ensuring they are counting and keeping track of children. Parents are being supervised by staff to ensure they are not distracted and are monitoring their assigned children and area of supervision. Door will remain closed and a door chime was installed.
