Comments:
Licensing Inspector arrived at 10:30 and departed at 11:45 AM to conduct a monitoring visit. At the time of the inspection, there were 10 children (ages infant through school-age) in care with two staff. Children were observed participating in center based learning activities. The sample size was one staff and three children's records. There was no medication being administered or stored on site.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on review of three children's records, it was determined that the facility did not ensure that it obtained documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Evidence:
Child #3, who was in care at the time of the inspection, did not have documentation of current immunizations on file.

Plan of Correction: Center director will contact the parents to obtain copy of the immunizations.

Standard #: 22VAC40-185-140-A
Description: Based on review of three children's records, it was determined that the facility did not ensure that each child had a physical completed by a physician on file within 30 days of enrollment.

Evidence:
All of the children's whose records were reviewed had been enrolled over 30 days. None of them had results of a physical examination on file.

Plan of Correction: Center director will contact parents to obtain copies of the physical examinations.

Standard #: 22VAC40-185-570-E
Description: Based on observation, it was determined that the facility did not ensure that all prepared infant formula was dated and labeled with the child's name.

Evidence:
There were two bottles of prepared infant formula in the refrigerator in the kitchen that were not labeled and dated.

Plan of Correction: Center director poured out the formula during the inspection.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of one staff record, it was determined that the facility did not ensure that all staff have a completed search of the central registry on file within 30 days of employment.

Evidence:
Staff #1, who was identified as an employee by the Center Director at the renewal inspection in March, did not have a completed search of the central registry on file.

Plan of Correction: Center director stated that the check had been requested and that she will follow up on it.
