Standard #: 22VAC40-185-40-I Description: Based on documentation review and staff interviews, the licensee did not ensure the center shall develop written procedures for injury prevention. Evidence: The center director confirmed there were not written injury prevention procedures available for review. Plan of Correction: The center director will reach out to other centers to create a new injury prevention plan specific to Middlesex.
Standard #: 22VAC40-185-550-C Description: Based on documentation review and staff interviews, the licensee did not ensure the center's emergency evacuation and shelter-in-place procedures/maps shall be posted in a location conspicuous to staff and children on each floor of each building. Evidence: One of three buildings used by the program, the "cottage" did not have an emergency evacuation and shelter-in-place procedures/maps posted. Children ages 4 years 9 months to age 6 were observed reading in the "cottage" during inspection. Plan of Correction: The center director will post those maps/ shelter in place will be created and in place by Friday.
