Comments:
An unannounced Monitoring Inspection was conducted on July 9, 2019, from approximately 1:50 pm to 5:10 pm. There were 56 children with five staff present and additional staff on site for administrative support. Children were observed playing small group games and during reading time. The sample size of records reviewed included six children's records and five staff records. Four medications were reviewed. Areas of non-compliance were noted on the violation notice. The results of the inspection were reviewed and verified during the exit interview with the center director. Please contact the licensing inspector with any questions (757) 404-0568.

Standard #: 22VAC40-185-40-I
Description: Based on documentation review and staff interviews, the licensee did not ensure the center shall develop written procedures for injury prevention. Evidence: The center director confirmed there were not written injury prevention procedures available for review.

Plan of Correction: The center director will reach out to other centers to create a new injury prevention plan specific to Middlesex.

Standard #: 22VAC40-185-550-C
Description: Based on documentation review and staff interviews, the licensee did not ensure the center's emergency evacuation and shelter-in-place procedures/maps shall be posted in a location conspicuous to staff and children on each floor of each building. Evidence: One of three buildings used by the program, the "cottage" did not have an emergency evacuation and shelter-in-place procedures/maps posted. Children ages 4 years 9 months to age 6 were observed reading in the "cottage" during inspection.

Plan of Correction: The center director will post those maps/ shelter in place will be created and in place by Friday.
