Classrooms not previously used by the SACC program that are currently being used for summer camp were measured today.
- Comments:

An unannounced monitoring inspection was conducted today. There were 5 children on site, with a total of 3 staff members assigned to their direct supervision. An additional 144 children and 24 staff members were off site attending a field trip to watch a basketball game. The children were observed playing games and creating art projects. Pleasant and friendly staff:child interactions were observed today. This inspection was completed between 10:00am and 11:40pm. Thank you for meeting with me today. Standard #: 22VAC40-185-130-A Description: Based on a review of children's health records, it was determined that two records were not complete. Evidence: Child record A and record F do not include the child's shot record. Plan of Correction: The shot records will be added to their files.
Standard #: 22VAC40-185-140-A Description: Based on a review of children's health records, it was determined that one record was not complete. Evidence: Child record F does not include a physical exam report. Plan of Correction: The physical will be added to the file.
Standard #: 22VAC40-185-60-A Description: Based on a review of children's records, it was determined that two records are not complete. Evidence: Child record A does not include: the phone number for the second emergency contact person Child record E does not include: the parent's home and work contact information; authorized pick ups information. Plan of Correction: These children do not attend this SACC program during the school year. The parents will be asked for the missing information.
Standard #: 22VAC40-185-520-B Description: Based on a review of children's sunscreen items, it was determined that some products were on site without all required information. Evidence: A total of 5 sunscreen products were not labeled with the child's name. A total of 4 sunscreen authorization forms did not include the correct name of the product to be applied to the child. Plan of Correction: The bottles will be labeled and parents will be asked to correctly complete the authorization forms.
Standard #: 22VAC40-185-520-D Description: Based on a review of the children's insect repellent products, it was determined that some were on site without all required information. Evidence: One bottle of insect repellent was not labeled with the child's name. One child's insect repellent was on site without an authorization form completed and signed by the parent. Staff do not record the application of insect repellent. Plan of Correction: The bug spray bottle will be labeled and the parent will be asked to sign the authorization form. We will begin recording when bug spray is applied.
