Technical Assistance:
Reviewed the requirement to document children's arrival and departure times.

Comments:
A Subsidy Health and Safety inspection was conducted today to review supplemental health and safety requirements. There were 5 children on site, with a total of 3 staff members. An additional 5 children arrived from summer school during this inspection. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: 22VAC40-665-520-B-11
Description: Based on a review of children's records, it was determined that six food allergy action plans were not completed as required. Evidence: A total of 4 children's records do not include a written Food Allergy Action Plan. Each of these children has a documented food allergy on file. A total of 2 children's records do not include the physician's signature on the Food Allergy Action Plan.

Plan of Correction: The action plans will be completed and parents will be asked to have them signed by the physician.
