Technical Assistance:
Technical assistance was provided blankets and pacifiers in a crib.

Comments:
An unannounced renewal inspection along with a self report investigation was conducted today from 10:00am-1:30pm. There were 63 children supervised by 13 direct care staff. The physical plant, programming, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, and playing outside. Diapering and hand-washing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, the outside fence is not maintained in a safe condition. Evidence: The side fence on the back playground does not come all the way to ground, leaving approximately a 2 inches gap between the ground and fence, posing a possible arm or leg entrapment.

Plan of Correction: I will look into finding something to block the space making it inaccessible to children.

Standard #: 22VAC40-185-340-A
Description: Based on interview with director and video observation, staff did not provide care and protection of a child. Evidence: Child #1 and child #2 were engaged in an physical interaction that lead to child #1 being bit several time in less than a 3 minute time period.

Plan of Correction: Management removed both staff from classroom and further action was taken.
