Comments:
An unannounced complaint investigation was conducted on 7/1/2019 from 11:30am to 1:00pm in response to a complaint that was received by the licensing office on 06/24/2019, regarding playground equipment. Observations were made at the center, all areas of the playground and equipment were inspected, and interviews with staff were conducted. Based on the information gathered during the investigation, the preponderance of evidence supports the allegation; therefore, the complaint was determined to be valid. The complaint related violation is identified on the Violation Notice. Description: Based on observation, areas and equipment of the center were not maintained in a clean, safe and operable condition. Evidence: On the preschool playground, two out of twelve tricycles were observed with loose bolts where the handlebars attach to the main frame of the tricycles, making them unstable and unsafe. A water table containing dirty standing water (approximately 1 1/2 inches deep), was observed with a lid, but accessible and within reach of children. One of three trash cans on the playground was also observed overflowing with trash. On the toddler playground, three of four riding toys were observed with cracked and broken wheels.

Plan of Correction: Ordered outside toys to be delivered before July 15th, water table empty and cleaned, broken bikes trashed, trash bins emptied.
