Comments:
An unannounced monitoring inspection was completed on 7/10/19. There were 42 children present in four groupings with eight staff directly supervising and additional administrative staff and volunteers present to assist if needed. Five newly enrolled children's records were reviewed and five staff records were reviewed. Children and staff records from the previous inspection were reviewed for correction of previous violations and were found to be corrected. There were no medications at the center. The inspector arrived at the center at 12:30 pm and departed at 5:00 pm. The children were observed finishing lunch and during nap. Infant activities were also observed. There was discussion with the administrative staff person about the following: attending Phase II orientation training (dates, how to register, etc.), using the most current forms (Sworn Statement for staff and immediate communicable disease reporting statement for parents), having copies of a disability plan for a child in care, staff transcripts for staff #4 to verify program lead qualifications.

Standard #: 22VAC40-185-260-A
Description: Based on observation of posted inspections and interview, the center failed to ensure that there was an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: 1. The posted fire inspection had a date of 6/20/18. The administrative staff and assistant director were asked and stated that an inspection had not been completed since that date.

Plan of Correction: The administrative staff person called the fire marshal's office during the inspection to request an inspection to be completed. The fire marshal's office personnel stated that an inspection should be completed by the end of next week.

Standard #: 22VAC40-185-550-M
Description: Based on review of records, the center failed to ensure that the written records of children's injuries contained documentation of all required information. Evidence: 1. Approximately 30 injury reports were reviewed for the months of April, May, and June 2019. Eighteen of the reports did not include the date, time, and how the parent was notified documented. This is required information to be documented for each injury report.

Plan of Correction: The assistant director/administrative staff will remind staff to complete the reports documenting all required information.
