Technical Assistance:
Technical assistance included obtaining a more inclusive description of the playground surfacing in use. The printout from the company was not specific in in descriptions of resiliency.

Comments:
An unannounced renewal inspection was conducted today from 10:30 am - 2 pm. There were 39 children present. All classes were within ratio. I observed children involved in a music and movement activity; building with Legos, listening to a story, playing outdoors and preparation for nap. Standard #: 22VAC40-185-70-A
Description: Based on a review of records today, not all staff had all required records. Evidence: Of 7 files inspected, Staff # 1 had no documentation of orientation as required.

Plan of Correction: She had been trained but we will use the state form to document the orientation.

Standard #: 22VAC40-185-270-A
Description: Based on observation during today's inspection, there was an area that was not maintained in a safe condition. Evidence: In the playground, an industrial strength mat (approximately 31 X 52) with a solid 1 3/4 inch height was placed at the base of the slide. Staff report that the mat, which was too heavy to easily move, was placed at the base of the bottom step to serve as a bottom stair for the slide because the height of the slide was altered when new surfacing was installed. As is, the heavy-duty mat presents a tripping hazard where it extends outward into the play zone.

Plan of Correction: We will put on some lime green florescent tape to make it more visible to avoid tripping and see how that works. If not, we will find another way to make it less of a hazard.

Description: Based on thermometer measurement today, the water temperature the sink in one room was too high. Evidence: The sink in the Teddy Bears Room measured 124 degrees F. The maximum safe temperature should be no higher than 120 degrees.

Plan of Correction: We will have someone from the church fix this. Today the children will use the sinks in the hallway bathrooms.

Description: Based on observation in one classroom today, cots/mats did not have at least 12 inches of space in between. Evidence: In one class, children were observed at naptime with approximately 2 - 5 inches apart.

Plan of Correction: Corrected during inspection.
