An unannounced monitoring inspection was conducted with the local agency beginning at 10:00 a.m. and ending at 11:45 a.m.regarding a self-reported incident for forbidden action: rough handling. Interviews were conducted. Documentation was reviewed. An additional site visit was conducted on April 26, 2019 to review documentation regarding the self-reported incident from 11:30 a.m. to 12:30 p.m. Given the information collected during the investigation, the report regarding forbidden action was not substantiated.
