Comments:
An unannounced renewal inspection was conducted on July 12, 2019 from approximately 10:00 AM to 12:30 PM to determine the center's compliance with licensing standards. The child to staff ratios were deemed in compliance with licensing standards. The following staff-to-child ratios were observed: Infants 3:11 and Toddlers 2:10. Lunch today consisted of fish sticks, crackers, corn, pineapple, and milk. The staff were observed to be engaged with the children in age appropriate activities and to be meeting the needs of the children in care. All areas of the facility including the classrooms, outdoor play areas, bathrooms, and the kitchen were inspected. The facility had the following posted: license, last inspection, emergency numbers, evacuation maps/procedures, daily schedule, and various parent information. The first aid kit and non-medical emergency supplies were inspected and found complete. Medications and authorizations were inspected. The last evacuation drill was conducted on 7/10/19. The last shelter-in-place drill was conducted on 1/7/19. The Emergency Preparedness plan is current. Health Inspection: 5/16/19. Fire Inspection: 7/1/19. There were multiple staff present who are certified in medication administration, daily health observation, First Aid, and CPR. 5 children's records and 5 staff records were reviewed. Violations:
Standard #: 22VAC40-185-160-C
Description: Based on a review of five staff records, the licensee did not ensure that one record contained documentation of an updated tuberculosis screening which is required at least every two years. Evidence: The record of Staff #1 showed a record of last having a tuberculosis screening on 4/21/17.

Plan of Correction: Administration will obtain the documentation for the TB screen and update the staff record.

Standard #: 63.2(17)-1721.1-B-2
Description: Based on review of the agent record, the center did not obtain a national fingerprint-based criminal history record check for the listed agent. Evidence: The record for Agent #1 did not contain documentation of a national fingerprint-based criminal history record check. A statement of eligibility based on a national fingerprint-based criminal history record check was required to be obtained prior to 9/30/2018.

Plan of Correction: The center will ensure the criminal history record check is submitted and will provide the results upon receipt. Documentation verifying the agent's fingerprinting will be forwarded to the licensing inspector.
