VENDOR AGREEMENT 9.7. Only parents or guardians of children receiving child care assistance are authorized to possess and swipe the VA-ECC card issued to them on the Vendor's POS machine or use the IVR system to report attendance. The Vendor shall not under any circumstance be in possession of or swipe any parents' or guardians' VA-ECC card or use the IVR system to report attendance; nor shall the Vendor ask the cardholder for his Personal Identification Number (PIN).

Comments:
Thank you for your assistance during this unannounced inspection. This inspection was initiated by an on-line complaint that was received in the Valley Licensing Office on June 18, 2019 regarding the allegation of the center keeping the subsidy VA-ECC cards of parents. The information gathered during this complaint supports the allegation and the complaint is determined to be valid. Standard #: 22VAC40-665-500-A
Description: Based on observation and staff interview, the center failed to ensure compliance with the terms of the vendor agreement. Evidence: 1. Six subsidy VA-ECC cards were found in a cup inside a white cabinet. 2. Staff #1 confirmed the cards were in the cup inside the white cabinet.

Plan of Correction: Spoke with parents letting them know they can not leave their cards at the center. They have their cards.
