Comments:
An unannounced mandated inspection was conducted today from 11:45am until 2:45pm. There were 18 children present with five staff members. The entire physical plant was inspected. Children's records, staff records, emergency plans, menu's, evacuation drills and emergency supplies were inspected. The children were observed watching a video and preparing for lunch. Lunch consisted of Turkey Hotdogs, corn, fruit and milk. There were no medications on site today. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of 4 children's files, the center failed to maintain a separate record for each child enrolled. Evidence: 1) Child #'s file is missing the addresses of both emergency contacts and did not have documentation of a physical within 30 days of attendance. 2) Child #2's file is missing the addresses of both emergency contacts and does not have documentation of a work phone number for the father. 3) Child # 3's file is missing the addresses of both emergency contacts and does not have documentation of a physical within 30 days of attendance. 4) Child #4's file is missing the addresses of both emergency contacts.

Plan of Correction: I will get the information from the parents and place in children's files.

Standard #: 22VAC40-185-240-A
Description: Based on a review of three staff records, the center failed to ensure that will receive training by the end of their first day of assuming job responsibilities. Evidence: Staff #1, #2 and #3 do not have documentation of receiving training by the end of their first day of assuming job responsibilities.

Plan of Correction: I will complete orientation and place documentation in staff files.

Standard #: 22VAC40-185-330-B
Description: Based on inspection of the playground, the center failed to ensure that the mulch under and around playground equipment was resilient. Evidence: The areas in and around playground equipment had less than 1 inch of resilient surfacing material.

Plan of Correction: We are getting new mulch for the playground.

Standard #: 22VAC40-185-330-E
Description: Based on observation, the sandbox on the playground was not covered when not in use. Evidence: The sandbox was not covered.

Plan of Correction: We are cleaning out the sandbox, adding new sand and purchasing a new cover.

Standard #: 22VAC40-185-550-D
Description: Based on log review, the center failed to ensure that two shelter in place practicer drills will be conducted per year. Evidence: No shelter in place practicer drill has been conducted for the calendar for 2018 or 2019.

Plan of Correction: We will begin conducting shelter in place practicer drills.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, the center failed to ensure that central registry records checks will be updated every five years. Evidence: Staff #1 does not have a current Central Registry Record Check, last Central Registry check 11/10/2011.

Plan of Correction: I will submit Central Registry Record Check form.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, the center failed to ensure that the findings of the most recent violation notice for the facility was posted. Evidence: The last violation notice posted at the entrance of the facility is dated 07/31/2018, the last inspection was 03/06/2019

Plan of Correction: I will post the most recent violation notice.
