An unannounced monitoring inspection was conducted from 11:30am-12:45pm. There were 86 children and 19 staff present. A sample of 8 children's records, 4 staff records, emergency materials, and the physical plant were reviewed. The children were observed playing on the playground and eating lunch in the cafeteria. Standard #: 22VAC40-185-60-A Description: Based on a review of children's records, 2 out of 8 records reviewed were incomplete. Evidence: 1. Child #6 (start date: 6/24/2019) did not have an immunization record on file. 2. Child #7 (start date: 6/24/2019) did not have an immunization record on file. Plan of Correction: We will ask the parents to bring the immunization records in.
