Comments:
A mandated Renewal inspection was conducted today, 7/15/19. There were 69 children and 15 staff present during this inspection. Children were observed transitioning to chosen activities, making edible play-doh, collaborating for a play, and participating in gross motor activities in the gym. Reviewed 5 children's files, 5 staff files, medications and authorizations, attendance and physical plant. Violations of the Standards are recorded on the Violation Notice. Standard #: 22VAC40-185-130-A
Description: Based on children's record review, the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: Child#3 (start date 12/5/18) did not have documentation of immunizations available in their file for review.

Plan of Correction: I'll call tomorrow and get it scanned to me

Standard #: 22VAC40-185-140-A
Description: Based on children's record review, each child did not have documentation of a physical exam within 30 days of the first day of care. Evidence: Child #3 (start date 12/5/18) did not have documentation of a physical available for review.

Standard #: 22VAC40-185-60-A
Description: Based on children's record review, the center did not maintain documentation of child updates and confirmation of up-to-date information in the child's record. Evidence: Child#2 (start date 6/1/18) did not have documentation confirming the information on file was up-to-date. Last up date was start date.

Plan of Correction: Ill have the parent do this tonight

Standard #: 22VAC40-185-510-A
Description: Based on children's record review, long-term prescription drug authorization did not contain written authorization from the child's physician and parent. Evidence: Child#1 (age 6) medication authorization for Benedryl and Epi-Pen Jr were expired. 5/21/18

Plan of Correction: Ill give mom the form tonight and have her take it to the doctors to update.
