An announced initial inspection was completed today. This was completed as part of a change in ownership of the BASIS Independent licensed program at 8000 Jones Branch Drive in McLean, Virginia. The physical plant, 5 staff files, 5 children's records, fire and health inspection, staff training, children's activities, and emergency procedures were inspected between 11:00-12:40pm. There were 23 children under the supervision of 5 staff as part of the Ooey Gooey Science Camp and Ocean Explorers camp. Today, the children made ice cream, ate lunch, read books and rested during naptime. The children bring their lunches from home each day they come to the center. The interactions between the children and staff were seen to be positive and constructive in nature. There is currently 1 staff certified in MAT (medication administration training). Standard #: 22VAC40-185-70-A Description: Based on file review, 3 out of 5 staff records do not have all required information on file. Evidence 1. Staff #2 and #3 do not have documentation that two or more references as to character and reputation as well as competency were checked before employment. Staff #2 has 1 reference check and Staff #3 do not have any checks on file. 2. Staff #2 does not have documentation to verify that the individual possesses the education required by the job position. Plan of Correction: We will get this documentation and keep it in their respective records.
