An unannounced inspection was completed on 07/15/2019 from 9:45 A.M.-2:30 P.M. At the time of the inspection 47 children were in care with eight staff and two administrators present. Five children records, five staff records, three medications and required documentation, fire inspection, health inspection, over the counter topical ointments and required documentation, five injury reports, emergency evacuations, first aid kits, emergency supplies, playground, and posted required information were reviewed. Children were observed during AM snack, handwashing, swim and water play, transitions time, art activity, circle time, music and movement, lunchtime and naptime. Standard #: 22VAC40-185-270-A Description: Based on observation, the center failed to maintain all equipment in a safe and operable condition. Evidence: 1. On the upper playground on the large climbing structure there is a board that is splintered and cracked at the top and bottom of the board. 2. On the bottom of the board there is a nail that is protruding out from the structure. 3. Staff verified that the board was splintered and cracked, and that there was a nail protruding out on the bottom of the board. Plan of Correction: Administration will notify the owners to have a new board purchased and replace the old board.
Standard #: 22VAC40-185-330-B Description: Based on observation and interview the center failed to ensure that the resilient surface is maintained as required. Evidence: 1. On the upper playground there is grass and weeds growing up through the mulch resilient surface in the fall/use zone of the the green slide. 2. Staff verified that there is grass and weeds growing up through the mulch resilient surfacing on the playground. Plan of Correction: Administration will work with the staff and start getting the fall zone weeded and maintained.
