Comments:
An unannounced complaint inspection was conducted in response to a complaint that was received by the licensing office on 5/6/2019. Interviews were conducted with staff relating to the allegation of meals being served and ratio. The preponderance of the evidence gathered during the investigation does support the allegation, so that complaint is determined to be " valid" .

Complaint related: No
Description: Based on observation, the center failed to ensure that all chemicals were locked. Evidence: 1. On 6/10/2019 the inspector observed unlocked chemicals sitting on the cabinet in the yellow room. There were accessible to the children in care.

Plan of Correction: The owner had the teacher to remove and lock the chemicals during the inspection.

Complaint related: Yes
Description: Based on interviews with staff, the center failed to ensure that ratio was met for two a year old children. Evidence: 1. Verified through interviews that on May 7 and on May 8th there 14 children with one staff person from approximately 2:30 P.M. to pick up time of the children. There were one child two year of age, four children ages 3-4 years of age, eight children school age (6-12 years of age).

Plan of Correction: Owner has changed schedule around to ensure that the correct number of staff if at the center and ratio is met during operating hours.

Complaint related: Yes
Description: Based on observation and interviews with staff, the center failed to ensure that all food served to the children met the USDA food requirements. Evidence: 1. Based on interviews with staff # 1, and 2 and owner, the children were served juice that was not 100% instead of milk with lunch. 2. Lunchables were served to the children for lunch on May 6th. The lunchable did not meet the USDA required for lunch sesrved to the preschool children.

Plan of Correction: The owner has meet with a USDA food person and has developed a new menu to be used.
