The standards related to food and nutrition related to serving snack and USDA requirements were reviewed.
- Comments:

An unannounced Monitoring Inspection was conducted on July 16, 2019, from approximately 12:45 pm until 5:15 pm. Upon entrance there were 20 children and 5 staff present with children. The record review sample consisted of 5 children's records and 3 staff records. Children were observed during snack time, working on an art project and during free play. The areas of noncompliance are noted on the violation notice. The results of the inspection were reviewed and verified by the Program Director on this date. Standard #: 22VAC40-185-140-A Description: Based on review of 5 children's records and staff interviews, the licensee did not ensure each child shall have a physical examination by or under the direction of a physician within one month after first attendance: Evidence: The record for child #2 did not include a physical. First date of attendance was 3-11-2019. The missing information was confirmed by the Center Director. Plan of Correction: Center Director will have parents hand in the physical.
Standard #: 22VAC40-185-260-A Description: Based on documentation review and staff interviews, the licensee did not ensure the center shall provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: The most recent fire inspection available for review is dated 4-27-18 and the center doe not have available for review an updated annual fire inspection. Plan of Correction: The Center Director will continue to contact Fire Marshall about retrieving the inspection report.
Standard #: 22VAC40-185-570-A Description: Based on observation and staff interviews, the licensee did not ensure that when a child is placed in an infant seat or high chair, the protective belt shall be fastened securely. Evidence: In the infant room child #1 and child #2 were observed during feeding in a feeding chair/ seat. The chair was placed on the ground and the protective belt was not fastened securely. The was confirmed by the staff in the infant room. Plan of Correction: Center staff will be trained on buckling the children into the feeding chairs.
