Comments:
An unannounced monitoring inspection was conducted today from 9:50 am until 12:45pm. There were 37 children present with 10 staff present. A complete inspection of the physical plant was conducted. Children's records, staff records, accident logs, evacuation drill logs, menu's, emergency supplies and medications were inspected. The children were observed on the playground, doing an experiment during large group time, hand washing and eating snack. Snack consisted of apples, cheese crackers and water. Standard #: 22VAC40-185-60-A
Description: Based on a review of 6 children's records, the center failed to maintain up to date information for each child enrolled. Evidence: 1) Child #1's file is missing documentation for addresses for both emergency contacts. 2) Child #3's file is missing documentation of a second emergency contact. 3) Child #5's file is missing documentation of both emergency contacts.

Plan of Correction: We will get this information, from parent and place in children's file.

Standard #: 22VAC40-185-290-3
Description: Based on observation, the center failed to ensure that electrical outlets will have protective covers that are of a size that cannot be swallowed by children. Evidence: 1) There were 2 outlets without protective covers in the 2's classroom hallway. 2) There was 1 outlet without a protective cover in classroom #1. 3) There were 3 outlets without protective covers in the activity room. 4) There are 2 outlet without protective covers in classroom #4.

Plan of Correction: Replaced during inspection.

Standard #: 22VAC40-185-330-B
Description: Based on an inspection of the playground, it was determined that the mulch was compacted and not resilient and did not measure at least 6 inches in depth underneath and surrounding the play equipment. Evidence: The mulch was compacted and therefore did not measure to be 6 inches in depth underneath and around play equipment.

Plan of Correction: We will rake the mulch and re-measure.
