Comments:
An unannounced renewal inspection was conducted today from 10:25am-1:45pm. There were 31 children (2-12yrs.) supervised by 6 direct care staff. The physical plant, 3 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in Three Little Pigs activities, building train tracks and bridges, playing with letter blocks, making rainbow fish, completing puzzles, and playing a bulls-eye paint game. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-160-C
Description: Based on a review of staff records, 1 of 3 staff records did not contain documentation of a follow-up tuberculosis screening at least every two years from the date of the initial screening. Evidence: Staff B's (date of hire: 9/13/2017) most recent documented TB test was dated 6/2/2017 (expired: 6/2/2019).

Plan of Correction: The employee will make an appointment to have the TB test updated.

Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, 1 of 4 children's records did not contain documentation of all required information. Evidence: Child #4's (start date: 8/10/15) record did not contain documentation of child updates and confirmation of up-to-date information in the child's record as required by 22 VAC 40-185-420 E 3. The most recent documented parental review of emergency information was dated 8/5/2015.

Plan of Correction: I will have the parent updated today.

Standard #: 22VAC40-185-290-3
Description: Based on classroom observation, 1 of 3 classrooms did not ensure that all electrical outlets had protective covers. Evidence: In the 3-5 year old classroom, 2 electrical outlets by the fish tank, and 1 electrical outlet by the bulletin board in the home living area, did not have protective covers.

Plan of Correction: Corrected during inspection.

Description: Based on a review of injury reports, 2 of 2 injury reports did not contain documentation of all required information. Evidence: 1. 2 of 2 injury reports did not contain documentation of the time and method of parent notification. 2. 1 of 2 injury reports did not contain documentation of the date of parent notification.

Plan of Correction: I have added the required lines on the injury/incident forms.
