Technical Assistance:
We discussed the requirements for cribs. The risk assessment for a violation was provided. We reviewed the requirements for written procedures, posting requirements, lead teacher qualifications, outdoor safety, diaper disposal, staff records, emergency planning and drills. We talked about the C.D.A. credential application process.

Comments:
Thank you for your assistance during today's announced initial inspection conducted from 10:00 AM to 5:10 PM. Today, I viewed the indoor and outdoor areas of the center, equipment, toys/materials, posted information, emergency procedures, written policies, three staff records, staff qualifications, staff training, menu, indoor and outdoor areas. The activity areas were measured for use with each age group. Although there is adequate space, the number of toilets in the center and the stipulations on the Certificate of Occupancy for the maximum capacity of persons present at any one time, will limit the capacity for children as eighty four children, only eighty of which can be preschool age. A six month conditional license will be recommended. Let me know if you need any assistance. 540/430-9259

Violations:
Standard #: 22VAC40-185-440-H
Description: Based on observation of the infant equipment, a review of the manufacture dates for cribs, and an interview with administrative staff, there were six cribs that were not in compliance with the federal requirements of the Consumer Product Safety Commission. Evidence: Nine cribs were seen in the infant care area of the center. All five of the metal Foundation cribs were labeled with manufacture dates of 2006 or 2007. A wooden crib, manufacturer unknown, was labeled with the manufacture date of 2009. There was not an accompanying manufacturer certificate, by the manufacturer or retailer, to demonstrate compliance with federal standards 16 C.F.R. Part 1219 or C.F.R. Part 1220. Only three cribs with manufacture dates after June 28, 2011 were in compliance.

Plan of Correction: An administrator immediately began moving the cribs out of the infant area and they will not be used. The administrator immediately arranged contact with the person who sold the five Foundation cribs to the center about the lack of compliance with safety standards. Four additional used cribs available for use will be checked to make sure they were manufactured after June 28, 2011 before they are brought to the center for use. The administrator began looking at new cribs to purchase.
