Comments:
A Subsidy Health and Safety inspection was conducted today to review supplemental health and safety requirements. There were 97 children in care, with a total of 12 staff members assigned to their direct supervision. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Violations:
Standard #: 22VAC40-665-510-B
Description: Based on a review of administrative documents, it was determined that the documented attendance does not include all required information. Evidence: The daily attendance does not include documentation of each child's arrival and departure time.

Plan of Correction: We will begin documenting arrival and departure times for children.

Standard #: 22VAC40-665-520-B-11
Description: Based on a review of children's Food Allergy Action Plans, it was determined that all required items were not included in children's records. Evidence: 1 - Child A and child C both have diagnosed food allergies documented in their enrollment files. Neither child has a completed Food Allergy Action Plan on file. 2 - The Food Allergy Action Plan for child B does not include the physician's signature.

Plan of Correction: All plans will be completed as required.
