Comments:
An unannounced monitoring inspection was conducted on 7/9/19 from 2:30m - 4:15pm. During the inspection there were 39 children ages nine months old through ten years old in care with 11 staff. Children were observed participating in various activities in the classrooms, and eating snack. Records were reviewed for five children and seven staff. There is no medication at the facility. Emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of five children's records and interview, it was determined that the facility did not ensure that they maintain and keep at the center a complete record for each child enrolled that contains all required information. Evidence 1. The record for child #2 did not contain the phone number for the child's physician. 2. The record for child #4 did not indicate whether or not the child had previously attended child care. 3. The record for child #5 did not indicate whether or not the child had previously attended child care, and did not contain the addresses for the emergency contacts. 4. Staff #8 confirmed that the records for child #2, child #4 and child #5 did not contain all of the required information.

Plan of Correction: The facility responded: The parents of each child will be asked to provide the missing information. All newly enrolled children's records will be reviewed prior to starting to ensure they are complete.

Standard #: 22VAC40-185-500-A
Description: Based on observation and interviews, it was determined that the licensee did not ensure that all children wash their hands with soap and running water before and after eating meals and snacks. Evidence: 1. The Licensing Inspector observe at least five children in the Preschool classroom leave the table after afternoon snack and start playing with various items without washing their hands. 2. When the staff present were asked if these children had washed their hands with soap and running water, they said they no.

Plan of Correction: The facility responded: Corrected during the inspection. As soon as it was pointed out that some of the children had not washed their hands after finishing snack, the staff re-directed the children to wash their hands. Staff will be reminded to make sure children wash their before and after all meals and snacks.

Standard #: 22VAC40-185-560-G
Description: Based on interviews, it was determined that the licensee did not ensure that when food is brought from home it is labeled with the child's name and date. Evidence: 1. The Licensing Inspector observed a bottle with formula in the refrigerator located in the Infant classroom that was not labeled with a child's name or the date. 2. Staff #7 acknowledged that there was a bottle that was not labeled with a child's name or the date.

Plan of Correction: The facility responded: Going forward, we will ensure that each bottle is labeled with the child's name and date.

Standard #: 22VAC40-185-570-C
Description: Based on a review of documentation and interviews, it was determined that the licensee did not ensure that the record of each child shall contain the child's formula and feeding schedule. Evidence: 1. There was not documentation at the center that contained the brand of formula and feeding schedule for each child in the Infant classroom. 2. Staff #8 confirmed that there was not a feeding schedule for each child who is on formula.

Plan of Correction: The facility responded: A list will be created which contains the brand of formula and feeding schedule for each child in the Infant classroom.
