Comments:
An unannounced renewal inspection was conducted today from 9:30am until 12:30pm. There were 64 children present with 15 staff. The physical plant, including the playground, children's files, staff files, emergency plans, menu's, injury logs, evacuation logs, attendance logs and emergency supplies were inspected. The children were observed picking vegetables from the garden, reading books, painting, hand washing and eating snack. Areas of non compliance were cited in the Violation Notice. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review 2 of 5 children's records, the center failed to maintain and keep up to date a record for each child enrolled. Evidence: 1) Child #1's file is missing documentation for a physicians phone number. 2) Child #2's file is missing documentation of a work number and employment for the mother and is missing the address for the second emergency contact.

Plan of Correction: I will get the information from parents and place in children's files.

Standard #: 22VAC40-185-330-B
Description: Based on inspection of the playground, the center failed to ensure that minimum depth of 6 inches of resilient material will be under equipment and surrounding playground equipment. Evidence: The mulch in areas of the playground measured between 2 inches and 3 inches around climbing structure and under swings.

Plan of Correction: Mulch is wet from recent storms, will dry it out and rake it and re-distribute it around play structure.
