Comments:
An unannounced monitoring inspection was completed on 07/19/2019 from 9:20 A.M.-3:00 P.M. At the time of the inspection 61 children were in care with thirteen staff and one administrators present. Six children records, seven staff records, one medication and required documentation, five injury reports, emergency plan, first aid kits, emergency supplies, health inspection, fire inspection, playground, and posted required information were reviewed. Children were observed during infant feeding, diapering, outside gross motor playtime, one-on-one infant and toddler interactions, behavioral guidance, free choice activities, transitions, handwashing, lunchtime, naptime, and a quiet time for schoolage children. Standard #: 22VAC40-185-140-A
Description: Based on a review of children's records, the center failed to obtain documentation of a physical examination within 30 days after the first day of attendance. Evidence: 1. Child #1's physical was dated 05/28/2019. Child #1's first date of enrollment was 03/11/2019. 2. Child #2's physical was dated 03/27/2019. Child #2's first date of enrollment was 01/02/2019. 3. Staff verified the the dates the physicals were obtained and the first day of enrollment.

Plan of Correction: Administration will track new enrollments and if not received with 30 day period services will be suspended until the physical form is received.

Standard #: 22VAC40-185-160-C
Description: Based on review of staff files, the center failed to ensure that at least every two years from the date of the initial screening a follow-up tuberculosis screening will be completed. Evidence: 1. Staff #4's last documented tuberculosis screening was dated 06/26/2017. 2. Staff verified the date of the most current tuberculosis screening on file as of 06/26/2017.

Plan of Correction: Staff will be sent to obtain a current tuberculosis screening within the next two weeks.

Standard #: 22VAC40-185-60-A
Description: Based on a review of children's files, the center failed to ensure that all required information was in a child's file. Evidence: 1. Child #3's file had no documentation of viewing proof of the child's identity and age. 2. Staff verified that Child #3's file did not contain documentation of viewing proof of the child's identity and age.

Plan of Correction: Staff will contact the parent and obtain documentation of child's identity and age.
