Comments:
An unannounced monitoring inspection was conducted today from approximately 12-3:30pm. There were 70 children in the direct care and supervision of 18 staff. A sample of 7 children?srecords, 5 staff records and 6 minor injury reports were reviewed. The children were observed reading books, playing on the playground, learning about pets and playing in water. 4 new staff members have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. If you have any questions, contact Kimberly.Sawyer@dss.virginia.gov.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, children's records were incomplete. Evidence: 1 of 7 children's records reviewed was missing required information. 1. Child A was missing 1 of 2 required emergency contacts, the address of one emergency contact and the first date of attendance.

Plan of Correction: I will give the file to the teacher and she will ask the parent to complete the file today.

Standard #: 22VAC40-185-70-A
Description: Based on record review, staff records were incomplete. Evidence: Staff C, whom is acting as the lead teacher for the school aged room, did not have a diploma or transcript on file to support her qualifications.

Plan of Correction: She will go home an her break and pick it up.

Standard #: 22VAC40-185-280-H
Description: Based on observation and interview, a purse was accessible to children. Evidence: 1. A black purse was found in a cubby where children keep there belongings. 2. Staff B stated that the purse belonged to her.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-185-550-E
Description: Based on record review, the center failed to maintain a record of the dates of the practiced drills for one year. Evidence: 1. One of two required shelter-in-place records for the last school year was missing. 2. Staff A stated they follow an August-August school year. 3. A shelter-in-place drill was conducted 10/2017. Staff A stated another one was conducted 3/2018. There was no documentation of this drill.

Plan of Correction: We will maintain the documentation.

Standard #: 22VAC40-185-550-M
Description: Based on record review, minor injury reports were missing required information. Evidence: 1 minor injury report was missing the method in which the parent was notified.

Plan of Correction: We will be more careful to fill in the whole form.

Standard #: 22VAC40-185-560-G
Description: Based on observation, food containers brought from home were not clearly dated and labeled in a way that identifies the owner. Evidence: There were 2 lunch boxes without a name and 4 without a date located in and on the cubbies in the hallway.

Plan of Correction: I'll send the parents a note and remind the teachers to label the lunch boxes.
