Comments:
An unannounced monitoring inspection was conducted 7/3/2019 from 9:25am-11:05am. There were 57 children directly supervised by 7 staff. The physical plant, outdoor playground areas, programming, menu, 7 staff records, 7 children?s records, 5 medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed reading books and completing various art and craft projects. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of age appropriate materials for the children. Areas of non-compliance are identified in the Violation Notice. In addition to the mandated monitoring inspection, a self reported allegation regarding behavioral guidance was also investigated. Interviews were completed on 7/3/19 and 7/8/19 and documentation was reviewed and a violation was found related to the self reported incident. Standard #: 22VAC40-185-130-A
Description: Based on review of 7 children's records, 1 child's record did not contain documentation of adequate immunization prior to the child attending the center. Evidence: 1. Child #7 (Date of Enrollment: 6/24/19)'s record did not contain immunizations.

Standard #: 22VAC40-185-400-B
Description: Based on child and staff interviews, a staff member did not provide behavioral guidance that is constructive in nature, or age and stage appropriate to redirect children to appropriate behavior. Evidence: 1. Interviews with Child #1 and Staff #1 confirmed the following: On Monday 6/24/2019 while the third grade group of children were lining up along the wall to go wash hands before snack, Staff #1 told the group "please don't touch the bulletin board". Child #1 (9 years old) took several popsicle sticks taped together and began sliding them along the bulletin board and underneath the papers stapled on. Staff #1 then grabbed Child #1's left wrist and she stated "I immediately knew I was in the wrong and released".

Description: Based on review of the emergency drill log, the center did not have documentation that monthly evacuation drills have been conducted. Evidence: 1. Dates of practice evacuation drills for May and June of 2019 were not on the emergency drill log.

Description: Based on review of 7 staff records, 1 staff record did not contain documentation that an out of state child abuse and neglect search request was submitted by the end of the 30th day of employment for staff who have resided outside the state of Virginia within the last 5 years. Evidence: 1. Staff #4 (Date of Hire: 4/16/2019) indicated on a Sworn Disclosure Statement dated 4/5/19 that she has resided outside the state of Virginia within the last 5 years. There was no out of state search request form located in the staff's file. 2. The director advised an out of state child abuse and neglect search request has not been submitted.
