Technical Assistance:
Consultation was given on Daily Health Observation certification is good for 3 years.

Comments:
An unannounced renewal study was conducted today from 11:30-12:25. Eighteen children were present with four staff. Observed children finishing snack/lunch, hand washing, reading, free play and transition to a special visitor (librarian). Reviewed as ample of children's records, medication, physical space and emergency preparation. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-60-A
Description: Based on record review, one of four children's records was incomplete. Evidence: Child #1 was missing two complete emergency contact addresses.

Plan of Correction: Center administration will request information from the parent.

Standard #: 22VAC40-185-240-D-5
Description: Based on staff statements, there was not at least one staff member on duty who has obtained, within the last three years, instruction in performing the daily health observation (DHO) of children. Evidence: None of the four staff present during the inspection had current DHO certification.

Plan of Correction: Assistant Director has registered for the DHO and will complete certification next week.

Standard #: 22VAC40-185-270-A
Description: Based on observation, an area of the center is not maintained in a clean, safe or operable condition. Evidence: 1. The door leading into the girls bathroom has peeling paint along the bottom of the door. 2. Two picnic tables have peeling green paint

Plan of Correction: Camp Supervisor will request that these items be corrected.

Standard #: 22VAC40-185-500-B
Description: Based on observation and staff statement, disposable diapers are not disposed in a leakproof or plastic-lined storage system that is foot operated. Evidence: The current foot operated trash can to dispose soiled diapers does not work and must be used by lifting the lid by hand.

Plan of Correction: Broken trash can was replaced with new foot operated trash can.

Standard #: 22VAC40-185-510-C
Description: Based on medication review, an authorization form does not authorize Arlington County Parks and Recreation staff to administer an emergency medication. Evidence: Child #2's long term medication authorization gives a private preschool the permission to administer the medication to the child rather than the Arlington staff.

Plan of Correction: Staff requested that parent update authorization.
