An unannounced complaint investigation regarding the physical plant was conducted today. The Inspector conducted observations of the physical plant, staff interviews, and a review of administrative documents today. After careful review of all available information, this complaint was deemed not valid. An unannounced monitoring inspection was also conducted today. There were 97 children in care, with a total of 12 staff members assigned to their direct supervision. Children's records, staff training, children's medications, administrative documents, emergency preparedness items, and the physical plant were inspected today. The children were observed attending summer camp activity classrooms which were supplied with a large variety of age-appropriate activities. The children were served turkey and cheese bagel sandwiches, black bean salad, orange slices and milk for lunch today. This inspection was completed between approximately 10:30am and 1:00pm. Standard #: 22VAC40-185-130-A Complaint related: No Description: Based on a review of children's health records, it was determined that one record does not include an immunization record. Evidence: Child record C does not include documentation of a current immunization record. This child has already entered care. Plan of Correction: I will call the child's base school to get a copy of his health records.
Standard #: 22VAC40-185-60-A Complaint related: No Description: Based on a review of children's records, it was determined that two records were not complete. Evidence: Child record C does not include "authorized pick ups" information. Child record F does not include: the complete employment contact information for each parent; the child's documented date of enrollment. Plan of Correction: The parents will be asked for the missing information.
Standard #: 22VAC40-185-510-C Complaint related: No Description: Based on a review of children's medications, it was determined that one authorization has not been completed as required. Evidence: The authorization form for one child's long term medication does not include the physician's signature. Plan of Correction: The parent has already been asked to have the doctor sign the form. I will follow up with the parent.
Standard #: 22VAC40-185-510-E Complaint related: No Description: Based on a review of children's medications, it was determined that one non-prescription medication was not labeled as required. Evidence: One child's non-prescription medication was not labeled with the child's name. Plan of Correction: The bottle will be labeled today.
