The licensing inspector initiated an Unannounced Complaint Inspection in the center on July 12, 2019, continued on July 17, 2019 and completed on July 22, 2019 in response to a complaint received by the licensing office regarding Programs and Special Care Provisions/Emergencies. Review of center documentation, interviews with staff members and observations were conducted. Based on information gathered during the investigation, there was not a preponderance of evidence that supports the allegation of Programs, however there was a preponderance of evidence to support the allegation regarding Special Care Provisions/Emergencies therefore the complaint is determined to be Valid. Areas of non-compliance are noted ion the Violation Notice. The results of the inspection were reviewed the the Center Director during the exit interview. Description: Based on observation, documentation review and staff interviews, the center did not ensure a record of administering diaper cream shall be kept that includes the child's name, date of use, frequency of application and any adverse reactions. Evidence: In the Transitional Toddler room, the center staff and center documentation confirmed when diaper cream was administered there was not a record maintained of administering the diaper cream as required.

Plan of Correction: The center director will retrain staff and follow up with staff during monthly staff meetings.

Description: Based on documentation review and staff interviews, the licensee did not ensure the center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence and include all required information. Evidence; One injury report was reviewed and the following required information was not included on the report 1) the time the injury occurred 2) the date and time parents were notified 3) a staff and parent signature or 2 staff signatures 4) any future action to prevent recurrence and documentation on how parent was notified

Plan of Correction: The center director will retrain staff at the August 1st meeting and then follow up with staff during monthly staff meetings.
