Technical Assistance:
Technical assistance today included record keeping and physical environment.

Comments:
An unannounced monitoring inspection took place today from 10:15 am - 2 pm. There were children 62 children present in 6 classes. All rooms were within ratio. I observed children involve in dramatic play; building with blocks, coloring; eating lunch and preparation for naptime. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, it was found that some information was missing. Evidence: A search of the files of Child # 1, 2, 3, 4, and 5 found that there were no emergency contact addresses as required.

Plan of Correction: We will make sure this information is complete.

Standard #: 22VAC40-185-270-A
Description: Based on observation during today's inspection, there were some areas of the center not maintained to a safe and operable condition. Evidence: Indoors: The door from the kitchen was broken, with some areas of shredded wood and a broken metal hinge, leaving the door unable to close. A light fixture in the ceiling of the hallway was missing a cover. Outdoors: There was a broken area in the lattice of the blue wooden gate near the shed. There was a container of propane in the fence next the the air conditioning unit, and some dangling cables between the entrance to the playground & entrance to the school. The newly installed fence had a gate handle that didn't easily open.

Plan of Correction: We will send photos to the church so these things can be replaced. The people who put up the fence are coming back this weekend to fix the outdoor items.

Standard #: 22VAC40-185-420-E-3
Description: Based on a review of children's records today, there was not documentation of annual updates of children's emergency contact information as required. Evidence: Of five files reviewed, none had documentation of an annual update.

Plan of Correction: This is will be documented.

Standard #: 22VAC40-185-510-A
Description: Based on a review of medication authorizations today, one authorization was expired. Evidence: The medication for Child # 2 expired 7/8/19.

Plan of Correction: We will contact the parent for an updated authorization form today.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on a review of records today, it was found that two staff members who lived out of state in the past five years did not have results of the background searches as required. Evidence: Staff # 2, who lived in another state in the past five years, did not have documentation that the search of the child protective registry was sent for. (date of hire: 5/19)

Plan of Correction: We will send this out today.
