22VAC40-185 STAFFING AND SUPERVISION.
An unannounced monitoring inspection was conducted to investigate a self-reported incident regarding supervision beginning at 9:55 a.m. and ending at 11:15 a.m. Documentation was reviewed. Staff interviews were conducted. Given the information collected during the investigation, noncompliance with the child day center standards were noted. Standard #: 22VAC40-185-340-A Description: Based on staff interviews, staff failed to ensure the care and protection of all children in care. Evidence: On July 12, 2019, at approximately 3:35 p.m. Child #1, age three years old, was left alone in the classroom by Staff A. When located approximately 10 minutes later by Staff B, the child was visibly upset and in tears. It took the child several minutes to regain composure after being located. Plan of Correction: The staff member was retrained on July 18, 2019 and remaining staff members will be retrained by July 26, 2019. Ongoing safety and supervision trainings will be provided during monthly staff meetings.
Standard #: 22VAC40-185-340-F Description: Based on staff interviews and review of documentation, the center failed to ensure that all children under the age of ten years old were within actual sight and sound supervision of staff. Evidence: On July 12, 2019 at approximately 3:35 p.m., Staff A transitioned the Preschool II classroom to The Brook multipurpose room. Child #1, age three years old, was left alone in the classroom for approximately ten minutes before being found by Staff B at approximately 3:45 p.m. Plan of Correction: The staff member was retrained on July 18, 2019 and remaining staff members will be retrained by July 26, 2019. Ongoing safety and supervision trainings will be provided during monthly staff meetings.
