Comments:
An unannounced renewal inspection was conducted with center staff on July 18, 2019 to determine compliance with the licensing standards. The children were engaged in a variety of activities including lunch time, watching an age appropriate movie, and having rest time. A group of 17 school age children were on a field trip to Chuck E Cheese. Lunch served today: turkey and cheese sandwiches, green beans, fresh pears, and milk. The last health inspection was conducted 11/14/2018. The last fire inspection was conducted 6/21/2019. Evacuation drills have been conducted monthly; the last shelter in place drill was conducted 1/11/2019. Two agent files and one new staff file were reviewed- please see Supplemental Summary for details. Ten children's files were reviewed and are in compliance. Standard #: 22VAC40-185-510-D
Description: Based on review of three medications, the facility failed to have documentation of written permission from each parent to administer medication to children. Evidence: there was no written authorization from the parent to administer one medication to Child B. Child C has an allergy action plan, but does not have written permission for the facility to administer the medication to the child.

Plan of Correction: The medication for Child B will be returned to the parent today. The facility will have the parent fill out the written permission for Child C today.

Standard #: 63.2(17)-1721.1-B-2
Description: Based on review of two applicant files, the facility failed to have documentation of fingerprinting for each applicant or agent. Evidence: Agent 1 did not have documentation of a fingerprinting check on or before 10/1/2018.

Plan of Correction: Agent 1 will submit to fingerprinting as soon as an appointment can be made with Field Print.
