Comments:
A Subsidy Health and Safety inspection was conducted on 07/23/2019 from 9:50am to 1:40pm to review supplemental health and safety requirements. The children were observed playing in centers, creating crafts, participating in water play, dancing and eating lunch. A formal observation was completed with the infant and two year old classrooms. Lunch consisted of sausage and cheese biscuits with jelly, peas and carrots, pineapples and milk. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today. Seven children?s records and six employee records were reviewed during this inspection. The center administers medication; however they did not have any being given at this time. Thirteen injury reports were reviewed as well as the center's injury prevention plan. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 07/15/2019 Last shelter-in-place drill: 06/14/2019 Last fire inspection: 08/16/2018 Last health inspection: 03/13/2019 Last lockdown drill: 07/18/2019 Today, the following child to staff ratios were observed: Infants - 9:3 Toddler ? 7:2 Twos - 7:1 Twos and Threes - 16:2 Preschool and Pre-K - 20:2 School Age - 12:1 A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. One Violation was found during the supplemental subsidy health and safety inspection and is identified on the violation notice. Description: Based on record review, the vendor did not ensure that two of seven children's records contained a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: 1. The record of child #2 (start date: 12/20/2018) did not contain documentation of a written allergy care plan signed by a physician. The child's record indicated that the child had a diagnosed food allergy. 2. The record of child #4 (start date: 6/05/2019) did not contain documentation of a written allergy care plan signed by a physician. The child's record indicated that the child had a diagnosed food allergy.

Plan of Correction: Children will be placed on suspension until documents are submitted.
