Comments:
An unannounced renewal inspection was completed on July 22, 2019 from approximately 10:15a.m. to 1:15p.m. Thirty children were in care and eight staff were on premises during the inspection. Staff and children were observed in classrooms and on the playground engaged in activities to include activities for infants, free outside play, lunch time, hand washing, and nap time. Interviews and interactions were conducted and medications were reviewed. Eight children?s records, five staff records and one corporate member record was reviewed. The Owner/Director was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee.

Standard #: 22VAC40-185-130-A
Description: Based on a review of children?s records, the licensee failed to obtain an immunization record for a child before the child attended the center for one of eight children. Evidence: 1. Child #5's record, enrolled on August 20, 2018, contained documentation of immunizations dated August 27, 2018 and therefore not within the required time frame. 2. Administrator #1 acknowledged this immunization record was not obtained by first day of attendance and indicated this was an oversight.

Plan of Correction: The Director/Owner will continue to follow the requirements of the Standards in which immunization records are obtained prior to a child's first day of attendance.

Standard #: 22VAC40-185-510-N
Description: Based on a review of medications, the licensee failed to notify a parent after a medication authorization expired that the medication needed to be picked up within 14 days, and then failed to dispose of medications that were not picked up.
Evidence: 1. Prescription asthma medication was observed for Child #6, over the counter allergy medication and an allergy cream were observed for Child #7, and over the counter skin cream was observed for Child #8. Staff #7 reported in an interview that Child #6 has not been in care for about three weeks, Child #7 has not been in care since the first week of June 2019, and that Child #8 has not been in care since about May 2019. Therefore, these medications were not disposed of as required. 2. Administrator #1 acknowledged these medications had not been disposed of as required and indicated she was not aware these medications were at the center.

Plan of Correction: The Director/Owner will create a process to ensure that medications are disposed of property.

Standard #: 22VAC40-185-550-M
Description: Based on a review of injury records, the licensee failed to maintain all required elements of a written record of children's serious and minor injuries in which entries are made the day of occurrence. Evidence: 1. Fifty-seven injury reports from May 2019 to July 2019 were reviewed. Thirty-eight lacked the time notification was given to the child's parent as required. 2. Administrator #1 acknowledged these reports were missing this required information.

Plan of Correction: The Director/Owner already retrained all staff at the beginning of July 2019 on the required elements of injury reports after noticing herself that this information was missing from injury reports.
