Comments:
An unannounced renewal inspection was conducted today, July 23, 2019 to determine the center's compliance with licensing standards. The inspection was initiated at 09:00am and concluded at 1:30pm. Upon the inspectors arrival, five children were observed participating with strech-n-grow, an exercise/movement program in the cafeteria. The Infant and toddler classrooms were combined. Children were napping, being feed and looking at picture flash cards. The two year old class, pre-k class and school age children were also combined. Children were playing with toy cars and coloring. Later children transitioned to their individual classrooms. The school age and pre-k children painted their own emoji's design onto canvases. Lunch time was also observed. Today's lunch consisted of ham and cheese sandwich, mixed vegetables, oranges, blue raspberry sorbet and milk. After lunch children had bathroom breaks and prepared for nap. All areas used for child care were inspected to include the first and second floor hallways, bathrooms and classrooms, and cafeteria. The playgrounds area was not inspected due to inclement weather. The facility had the following information posted: license, last inspection notice, emergency numbers, daily schedules, evacuation map/procedures, and menu. The first aid kits for each floor of the building along with non-medical emergency supplies were reviewed and found complete. The inspector interacted with staff and children in care. Seven staff records and five children's records were reviewed. Staff on site have DHO training, MAT, CPR and first aid training. Per the Director; medications are not being administered at this time. The most recent evacuation drill was documented on 06/24/2019. One of the two required shelter-in-place drills for 2019 was conducted on 05/14/2019. A lock down drill had been completed on 06/14/2019. Today's Census: Infants & creeper combined: 11 infants/toddlers with 2 staff. (later: 4 Infants with 1 staff, 8 creepers with 2 staff) Two's/Pre-school/ school age: 10 children with 2 staff. (two's class: four children with 1 staff and 1 volunteer, pre-K/school age: 6 children with 1 staff) Health Inspection 01/11/2019 Fire inspection 12/20/2018 Violations cited during the licensing period were reviewed for corrections, there were three repeat violation cited during today's inspection. Standard #: 22VAC40-185-140-A
Description: Based on review of five children's records, the center did not obtain documentation of a physical examination before the child's attendance or within one month after attendance. Evidence: 1. The record of child #2, enrolled 11/13/2017, did not have documentation of a physical examination on file exceeding 30 days from enrollment. 2. The record of child #3, enrolled 12/31/2018, did not have documentation of a physical examination on file exceeding 30 days from enrollment.

Plan of Correction: Per the Director: I just called the parents and requested a copy of their child's physical. They will provide a copy for the records.

Standard #: 22VAC40-185-350-C
Description: Based on observation and interview, the center did not ensure when children are regularly in ongoing mixed age groups, the staff to children ratio applicable to the youngest child in the group applied to the entire group. Evidence: 1. Staff stated the infants and toddler are regularly combined in the morning from 7:00 am until approximately 9:30 am when additional staff arrive. 2. Eleven children aged two months through twenty three months were observed being supervised by two staff in the creeper classroom from 9:05 am until 9:30 am when a total of three staff was required. 3. The applicable staff to child ratio was one staff member for every four children.

Plan of Correction: Per the Director: Today we had a staff call out and our extra staff went into another classroom. We are hiring additional staff to have a floater who will go into classrooms when needed. We will also look at the number of children in attendance and adjust staff hours as needed to meet the required ratios.

Standard #: 22VAC40-185-420-E-1
Description: Based on review of documentations and interview, the center did not maintain a posted daily record for each infant that contained all required information. Evidence: 1. The daily Infant record sheet did not contain the amount of time an infant, who is awake and cannot turn over by themselves, spent on their stomachs. 2. Staff #3 stated infants had tummy time but it was not recorded on the infant daily sheets. 3. Child #6, two months in age, did not have documentation of having time on his stomach.

Plan of Correction: Corrected while the inspector was on site. The Director created a new daily infant record with a space to document tummy time. Per the Director: I will instruct staff to record tummy time daily.

Standard #: 22VAC40-185-570-C
Description: Based on review of documentation and interview, the center did not maintain a record of each child on formula that contained the brand of formula and the child's feeding schedule. Evidence: 1. Child #6, who is on formula, did not have a record of the brand of formula and the child's feeding schedule. 2. Staff #3 stated she did not have a record of the brand of formula or feeding schedule for child #6.

Plan of Correction: Corrected while the inspector was on site. The Director added the formula brand and feeding schedule of child #6 to the record.
